How to Get a SMART® Health Card for tests or vaccinations.
You might receive a paper or digital SMART Health Card with your test or vaccination record from any organization that has your clinical information, such as a pharmacy, doctor’s office, or state registry.
If you are looking for a record of your COVID-19 immunization, many state immunization registries continue to offer those in the form of a SMART Health Card. Find your issuer here.
If you were vaccinated for COVID-19 at a retail pharmacy, you may be able to get a SMART Health Card by logging into that provider’s portal:
Full immunization records in the form of SMART Health Cards have begun to roll out in 2023. States that have implemented this can be found on the Docket website and currently include:
Alaska
Idaho
Minnesota
New Jersey
Utah
Wyoming
You may be also able to request a SMART Health Card from your health system:
If your healthcare provider uses MyChart (Epic):
Log into your MyChart account on the web or via app.
Go to the Menu and select COVID-19 under My Record.
Select QR codes to generate a QR code or Download/Export to generate a PDF copy of your vaccine results.
If your health system uses the Cerner HealtheLife app:
Log into your My Health account on the web or via app.
If your health system is issuing SMART Health Cards and is connected to CommonHealth (such as UCSD Health or UC Health), these records may directly appear in the CommonHealth app when you connect CommonHealth to your provider.
